Notice of exercise of the right of withdrawal

I/we hereby give notice that I/we wish to withdraw from the agreement concerning the following service:

• Service and service address
• Date the agreement was entered into
• Order reference, if available
• Name of the consumer or consumers
• Address, telephone number and email address
• Date on which the notice is sent

When submitted on paper, the form may be dated and signed. A signature is not required when the notice is sent electronically.

Deadline

To meet the withdrawal deadline, it is sufficient for the notice concerning the exercise of the right of withdrawal to be sent before the deadline expires. Keep documentation showing that the notice was sent.
